In the 4 state workflow shown below, you might want the workflow to move from a Rejected state  state to a more proactively named Triage state state.

You can use a trigger to listen for the state change event to the the Rejected state  state and set this trigger action to immediately transition to the Triage statethe Triage state.

The workflow document activity report will include the transition to the Rejected state and then the Triage state where your editors undertake any required updates.
